Does this replace backups?

No, and nothing does. Security stops most of what would damage the site; backup is what you use when something gets through anyway.

Will scanning slow the site down?

No. The scan runs from outside, the way a visitor sees the site, so nothing extra runs on your hosting account.

What if the site is already infected?

The first scan finds it and the basic plan cleans it once. If it keeps coming back, that is a hole rather than an infection, and support will help you find it.
